==== Absorption ==== Tizanidine is essentially completely absorbed with oral administration. The oral bioavailability of tizanidine is approximately 40%. It is subject to extensive first-pass metabolism and this is responsible for its incomplete bioavailability. The pharmacokinetics of tizanidine are linear across a dose range of 1 to 20 mg. The time to peak levels with tizanidine under fasting conditions is 1 hour and under fed conditions is 1.5 to 3 hours. The pharmacokinetics of tizanidine with oral tablet and capsule formulations are the same under fasting conditions but diverge and are non-equivalent under fed conditions. The pharmacokinetics of tizanidine also change when capsules are opened and it is sprinkled onto applesauce versus intact capsules.
== Chemistry == Enobosarm is a small-molecule (molecular weight = 389.3 g/mol) and highly lipophilic (predicted log P = 2.7–3.3) compound. Enobosarm and related SARMs like acetothiolutamide, andarine (acetamidoxolutamide; GTx-007; S-4), and GTx-027 were derived from structural modification of the arylpropionamide nonsteroidal antiandrogen bicalutamide. They are nonsteroidal arylpropionamides themselves and are close structural analogues of bicalutamide. This layer is transferred and from it RNA is extracted. RT-PCR is used to detect a selected expression of RNA. For example, Human placental lactogen (hPL) and beta-hCG mRNA are stable in maternal plasma and can be detected. (Ng et al. 2002). This can help to confirm the presence of cffDNA in maternal plasma.
== History == New Drug Applications (NDA) for eteplirsen and a similar drug drisapersen were filed with the US Food and Drug Administration (FDA) in August 2015. The Prescription Drug User Fee Act (PDUFA) goal dates for these were December 27, 2015 for drisapersen and February 26, 2016, for eteplirsen. Following FDA rejection of drisapersen, the agency announced a three-month time extension for its review of eteplirsen. The FDA panel decision was controversial because the FDA staff and the panel used a stricter standard of evidence than Sarepta and patient groups used. The FDA panel said that it was required by law to apply the standard of "substantial evidence" of effectiveness. This required randomized, controlled trials showing effectiveness of a meaningful clinical outcome, such as the ability to function in daily life. Sarepta and patient groups wanted to use the standard of historical controls, personal testimonies, and the presence of altered dystrophin in the body. On April 25, 2016, the Advisory Committee Panel voted against approval;. However, in June 2016, FDA requested for additional data from Sarepta to confirm findings of dystrophin production by eteplirsen. Janet Woodcock, director of the FDA's Center for Drug Evaluation and Research, overruled the panel, and FDA Commissioner Robert Califf deferred to her decision. Eterplirsen received accelerated approval on September 19, 2016. The European Medicines Agency reviewed the molecule in 2018, and refused to approve it.
